I did about 20 countess runs in nightmare and I get up to HEL runes often... So i started running it in hell and notices the drops got worse...usually tal and ort even eld.... what gives?|||From my experience, this has always been the case, even in previous patches.
I have the best luck on Nightmare difficulty. Since the release of the 1.13c patch the Countess seems to drop decent mid-level Runes. She usually drops 3 to 4 Runes, sometimes only one or two and rarely none. When I need El, Eld, Tir or other low-level Runes I do the Countess on Normal difficulty. Doing the Countess on Hell difficulty is a crap-shoot. Too much work and time, yielding little in return, unless you're very lucky.
For high-level Runes you'll just have to do like other players and search the known Rune-dropping areas, specifically on higher difficulty levels.
This information is based strictly on my 10 years playing D2 and not on any drop calculators or number-crunching info.
